PROOF
OF IDENTITY
Trainees
must provide documentation
which supports his/her true identity. The Logisafe Representative
must sight two documents verifying ID - one to contain a photograph. All documents must be originals; photocopies
are not acceptable.
Proof
of Identity Documents:

Current
Australian drivers license with photograph or 18+ card or equivalent
photo ID.

Current
Medicare card.
|
|
Birth
Certificate (extract will suffice)
|
Current
credit card, or account card from a bank, building society or credit
union.
|
|
Current Australian
passport
issued by the Australian Passport Office or an Australian passport that
expired within the last two years.
|
Pension
concession card, Department of Veterans affairs entitlement card,
Seniors Health card or any other current entitlement card issued by the
Commonwealth or State Government.

|
Current
overseas passport or overseas passport that has expired within the last
two years.
|
Passbook from a bank,
building society or credit union.
|
|
Australian
naturalization or citizenship document or immigration papers issued by
the Department of Immigration and Multicultural and Indigenous Affairs.
|
Telephone,
gas or electricity account up to one year old where name and address
match those on this application.
|
If the applicant is unable to provide sufficient documentary evidence a Statutory Declaration will be provided
and must also be completed.
Exception
for school students
School
students who cannot comply
with the ID requirements set out above must provide a
birth
certificate, passport or Australian Citizenship Certificate
(mandatory), and one other proof that may be:

letter (on school letterhead) from the School Principal

Student
bus pass
|
|
Student’s
library card
|
|
Current
credit card, account card or passbook from a banking institution
|
|
Mobile phone account
in
their name less than one year old
|
If
any of these
documents are being mailed, they will need to be certified by a
Justice of the Peace.
